Experience of Hospice in Home Through Tospital-hospice Care Team Connected with Community Health Center

Abstract
Background and Objectives: The purpose of this study was to provide hospice in home to terminal cancer patient and alternative proposal for activation efficient hospice in home through hospital-hospice care team connected with community health center.

Method: From April, 2008 to January, 2009, the hospital hospice team in Bucheon presented hospice in home connected with community health center.

Result: The patients discharged from our hospital were enrolled cancer patients stayed at home through a request to community health center and provided health care and the patients asked of medical service among enrolled patients in community health center were provided hospice in home by the hospital-hospice team through a request to hospital-hospice team. Through this system, the terminal cancer patients could be serviced constant and efficient health care under the reality of insufficiency of specialists in hospice.

Conclusion: In this study, we provided hospice in home to terminal cancer patients through hospital-hospice care connected with community health care for constant and efficient hospice in home. In the light of good satisfaction about constant health care and a role of volunteer, we can provide an alternative proposal for activation of hospice in home under the reality of insufficient hospice in home because of the institutional inertia.
